Forend with 2 picatinny rails and covers for the holes if you don't want to install them. Comes with instructions and adapters.

Fits mossberg, remington and Winchester..  make your research to see if it fits your 12ga. I couldn't install it because my version of the Moss 500 has a pined forend instead of an action slide assembly...
