Optima HP Extension +100mm

Was sent the incorrect gauge barrel extensions due to translation error from a shop in Italy. 

Adds 4" to your 20 gauge Beretta shotgun if it uses Optima HP chokes. 

So there's no confusion the extension is cylinder bore threaded on both ends.  It screws into your barrel then you screw your desired choke into the extension.
